
At the bottom of the Evaluation Main page are a set of commands. To create a new evaluation form, click on New. You will be brought to a page with a blank template and at this stage you can begin to create your form.
Form Heading
Each evaluation form has a heading that gives important information about the evaluation such as Title, who/what is being evaluated, and which printed form (RPT file) will be used. You can also select the evaluation to be generated daily, force the evaluator to answer all questions, or add another level of anonymity that would restrict this completed form from being displayed. You can also add Directions to your form similar to Exams.

By checking options to the right of the form heading, you can further specify how and when you want this form to be completed and displayed once it has been filled out.
Daily: checking this option signifies that this evaluation is to be generated and completed daily.
Force Answer All Questions: this option compels the evaluator to answer all questions before being allowed to Save the evaluation as complete.
Don't Show Completed Forms: this option gives the coordinator the ability to not allow whomever is being evaluated to view this form when it is complete.
Adding Groups
Before you can add questions, you need to create a Group. The group serves as an organizational unit for each set of questions. There needs to be at least one group per evaluation form.

Type in the name of your group in the space provided and click Add Group. In this example, the name of the first group will be Interpersonal Skills.

Once the group has been added, new options appear. Open/Close group will allow you to collapse your groups with questions. Move Group Up/Down will reorder the groups with questions, either up or down. Remove Group will remove the section and all of its associated questions.
Add Questions
After a group has been added, you can begin to add questions. Questions can be multiple choice (or scaled), comment or yes/no types. Text must be entered into the Question field in order to Save. Values can be added per question, and can be copied from the previous scaled question. Questions can also be limited to certain rotations using one master form.
Move Questions
Questions can be reordered within the same group. After you have inserted a question, to the left there will be a bullet and a color-shaded area. This is called the drag handle. When you click on it, you should see the entire question selected to be moved. The movement process may take a few seconds depending on the browser you are using or other technical factors.

Copy Question Headers
When you have entered several scaled questions, you only need to enter in the values for your scale once in the first question. For each subsequent question entered, you can click Copy Headers from Question Above when you are editing your question options. This will populate the scale according to what has been entered for the first scaled question.
